YHDP Intake & Assessment

Service Description

Welcome! This intake appointment is the first step in determining your eligibility for the Youth Homelessness Demonstration Program (YHDP). During this one-on-one meeting, we'll learn about your current housing situation, employment, education, and goals. We'll complete the required assessment, answer your questions, and discuss available resources that may include: • Rapid Rehousing (RRH) • Supportive Services Only (SSO) • Employment and job readiness assistance • Peer coaching and mentorship • Financial literacy and life skills • Housing navigation and referrals If you're between the ages of 18–24 and are experiencing homelessness or housing instability, this appointment can help connect you with the support you need.
